Background and Approach to Coaching
Thank you for choosing me, Kay Stevens, as your coach. I realize that working with me is a major decision, and you may have many questions. This document is intended to inform you of my policies, state and federal laws, and your rights. If you have other questions or concerns, please ask and I will try my best to provide all the information you need. My formal education consists of the following:

I am a Board Certified Coach specializing in research-based life, confidence, desistance, and emotional intelligence coaching services for teens and adults.

My relevant certifications and licenses include Board Certified Coach (BCC), Certified Youth Resilience Coach, and Fostering Success Coach II. I have over 180 hours of coach-specific training from International Coach Federation accredited coach-specific training providers and am in the process of attaining a PCC level ICF credential. Additionally, I am SafeZone and Mental Health First Aid certified.

My relevant continuing education includes Motivational Interviewing, Acceptance and Commitment Training, Mindfulness, Acceptance, and Positive Psychology, Fundamentals of Neuroscience, and Solution-Focused Interventions. My work is limited to coaching teens and adults in the following areas: life/personal development coaching, desistance and recovery coaching, career/employee/professional development coaching, leadership development coaching, identity-based exploration and development coaching, conflict coaching, and emotional intelligence coaching.

My professional experience includes over 10 years of progressive experience in the postsecondary education sector with primary responsibilities including coaching, training, mentoring, leading, interviewing, student support and crisis response, and program development. I’ve been a professional coach in the higher education field for 7 years and private practice for 3 years, with a primary focus on serving youth and young adults. I am the founder of Stevens Coaching LLC. I am also a leadership development administrator at a University, coaching and training student leaders as well as coaching and training full time staff in the areas of coaching and leadership development.

My eclectic background and training has enabled me to develop an approach to coaching that incorporates inclusivity, trauma awareness and resilience, emotional intelligence, and empowerment. I combine all of these methods in a coaching environment as a coach in an effort to help you or your child get unstuck, plan action steps, stay motivated, and achieve goals. While my approach uses some techniques drawn from psychology, I am not a therapist and I am not trying to practice psychotherapy using these techniques.  Many different therapeutic techniques can be adapted for coaches and coaching clients and I may use some of these in my practice. However, I will speak with you about referral to a licensed therapist if our work together indicates that you may benefit from that service in combination with coaching.

Nature of My Distance-Coaching Practice:
Prior to beginning coaching, you may have a free consultation with me by telephone or in person, if local to my area, to assess your needs and discuss my approach, so you can make an informed decision regarding working with me. During this consultation, I will explain how we will work together, discussing confidentiality, your rights as a client, and other nuances of working at a distance (if relevant) including your preferred coaching method (phone, Zoom, or in person; if local to my area) and a coaching schedule and frequency that works best for both of us.

If you agree to work with me, you will then book your coaching package through Paperbell and fill out and sign some basic forms (informed consent, intake survey, and client agreement). These signed documents and initial payment are due in order to schedule your session(s) and secure your coaching slot(s). The agreement is not complete until signed documents and initial installment are received. Upon receipt, you will receive an e-mail confirmation advising that we are set to begin coaching on the agreed upon start date.

*** If the client is a minor child, this informed consent, accompanying client agreement, and all other coaching documents requiring agreement to terms, conditions, and/or legal authorization must be completed by the child’s parent, guardian, and/or legal representative. 

Structure of Coaching Sessions:
During our sessions, you will be a very active participant in the coaching process. In addition to talking with me, you will select your own “homework” assignments and be asked to practice techniques learned through coaching sessions. Your commitment to doing this work outside our live sessions is crucial to your success. I expect that you will do all of the exercises prior to our live sessions together.

Insurance Issues:
I do not accept third-party reimbursement from health insurance carriers. I do not accept assignment of benefits, nor do I participate in managed care insurance plans (HMO’s and PPO’s).  If you have insurance that provides coverage for my services, I will gladly discuss the coaching services you receive from me if your insurance company calls me and you provide me with a release granting me the right to talk with them. I do not call to request authorizations. You are responsible for contacting your carrier, securing necessary forms, filling them out, and sending them back at your expense.

You are responsible for paying for all coaching services in full to me prior to submitting any insurance claims to be reimbursed by your insurance company.

Coordination of Care:
It is important that all healthcare providers work together. As such, if you would like me to share information about your coaching sessions, I will need your permission to communicate with your primary care physician, therapist, or both. In the event that you grant me this permission, please be advised that I will not initiate contact with them. They must request information directly from me. You will need to submit their contact information along with a note stating your consent to coordinate care directly to me in writing. Your consent is valid for one year. Please understand that you have the right to revoke this authorization, in writing, at any time by sending notice. If you prefer to decline consent, no information will be shared. Please initial beside your selection.

Confidentiality and Emergency Situations:
Your verbal communication and coaching records are strictly confidential except for (1) information shared with your insurance company to process your claims (if my services are covered by your insurance), (2) information you report to me about physical or sexual abuse (I am obligated to report this to the Michigan Department of Health and Human Services or like service in your state of residence), (3) when you sign a release of information to have specific information shared with your physician or therapist, and (4) when you provide information that informs me that you are in danger of harming yourself or others.

If the client is a minor, a summary of coaching sessions sharing overview content is available to parents/guardians in order to keep parents/guardians apprised of coaching progress. For example, an e-mail “recap” can be sent to the parent/guardian(s) with a CC: to the minor client, for transparency, including an overview of coaching topics (e.g., personality, strengths, self-concept, self-awareness, learning style, structuring tools for success, emotional intelligence, social intelligence, etc.). Detailed information of client communications to the coach through the coaching relationship is held in confidence unless it meets any one or more of the four exceptions noted above.    

If an emergency situation occurs for which you feel immediate attention is necessary, you understand that you are to contact the emergency services in your community (911). I will follow up those emergency services with coaching service and support.
