Plant-Based Diet Coaching Program: Terms and Conditions
This document outlines the terms and conditions governing your participation in the Priority Healthcare Plant-Based Diet Coaching Program (the "Program"). By enrolling in or participating in the Program, you acknowledge that you have read, understood, and agree to be bound by these terms.
1. Scope of services
- The Program provides guidance and support for adopting or enhancing a plant-based diet.
- It offers educational and informational resources, coaching sessions, and personalized support to help you achieve your plant-based dietary goals.
- Coaching sessions may occur in person, by phone, through video conference, or over email, depending on the agreed-upon package.
- The coach will provide ongoing support, education, and accountability as you create an action plan to meet and maintain your health goals.
2. Health and medical disclaimer
- The coach is not a dietitian, or therapist and does not diagnose, treat, or prescribe medications.
- The information and content provided during the Program are for educational purposes only and are not intended to be a substitute for professional medical advice, diagnosis, or treatment.
- Always seek the advice of your physician or other qualified health provider before making any changes to your diet or health regimen.
- Consult your physician about the suitability of a plant-based diet for your individual needs and discuss any potential interactions with medications you are taking.
- Do not disregard, avoid, or delay obtaining medical or health-related advice from your healthcare professional because of information received during the Program.
3. Client responsibilities
- You agree to take 100% responsibility for your own results and to actively engage in the Program, including participating in sessions, sharing relevant information, reflecting, listening, and identifying goals in collaboration with the coach.
- You agree to adhere to established appointment times.
- You agree to cancel or reschedule appointments with adequate notice (e.g., 24 hours) as outlined in the Program details.
- You understand that your experience and results are solely based on your level of participation, commitment, and honesty.
4. Confidentiality and data privacy
- All information shared by the client during coaching sessions will be kept confidential.
- Confidentiality does not extend to situations where the coach is legally obligated to disclose information, such as when there is a risk of harm to the client or others.
- Any client data collected will be stored securely and processed in accordance with applicable data protection laws, such as General Data Protection Regulation (GDPR) if applicable.
- You will be informed about the types of data collected, the purpose of collection, and your rights regarding your data.
5. Fees and payment
- Specific coaching fees, packages, and payment terms will be outlined in a separate agreement or on the Program website.
- Payment options may include upfront payment, monthly installments, or per-session payments.
- Details regarding cancellation fees and late payment policies will be clearly communicated.
6. Intellectual property
- All coaching materials, resources, and methods provided during the Program, including worksheets, guides, or other content, are the intellectual property of Priority Healthcare and are protected by copyright.
- You may use these materials for your personal coaching journey but are prohibited from reproducing, distributing, or sharing them without express written consent.
7. Limitation of liability
- The coach is not liable for any direct, indirect, incidental, or consequential damages arising from your participation in the Program.
- You acknowledge that you are responsible for your own decisions and actions resulting from the coaching sessions.
8. Termination
- Either party may terminate the coaching relationship with reasonable written notice, as outlined in a separate agreement or Program details.
- Details regarding early termination, including any applicable fees or outstanding payments, will be specified.
9. Governing law
- These terms and conditions will be governed by and construed in accordance with the laws of Georgia, U.S.A.
10. Modifications
- These terms and conditions may be updated from time to time. You will be notified of any changes, and your continued participation in the Program constitutes your acceptance of the revised terms.
