Health Coaching Client Agreement 
1. Scope of Services & Disclaimer
Health coaching is a collaborative, educational, and client-centered process designed to support personal insight, behavior change, and lifestyle development. Coaching services are informational and educational only and are not intended to diagnose, treat, prevent, or cure any disease or medical condition.
I am not a licensed physician, psychologist, registered dietitian, or mental health provider. Health coaching does not replace medical care, therapy, or advice from qualified healthcare professionals. Clients are encouraged to consult their physician or other licensed providers regarding medical concerns, medications, mental health treatment, or dietary changes.
Clients acknowledge that they are solely responsible for their health decisions, actions, and outcomes.
2. Client Responsibility
Clients understand that coaching is a voluntary process that requires active participation. Results depend on many factors, including personal effort, circumstances, and consistency. Clients retain full responsibility for their choices, behaviors, and well-being during and after the coaching relationship.
3. Confidentiality
All client information shared during coaching sessions is treated as confidential and will not be disclosed without written consent, except in the following circumstances:
  • If required by law
  • If there is a reasonable belief of imminent risk of harm to the client or others
  • If disclosure is necessary to comply with ethical or professional obligations
Coaching sessions are not considered legally privileged communication.
4. Payment, Scheduling & Cancellation
Coaching sessions are scheduled by mutual agreement.
  • Clients must provide at least 24 hours’ notice to reschedule or cancel a session.
  • Sessions canceled with less than 24 hours’ notice may be forfeited.
  • If fees are introduced in the future, payment terms, rates, and accepted methods will be communicated clearly in advance.
No refunds are guaranteed unless explicitly stated in writing.
5. No Guarantee of Results & Limitation of Liability
Health coaching does not guarantee specific outcomes. Individual results vary, and no promises or warranties are made regarding results, progress, or success.
To the fullest extent permitted by law, the coach shall not be liable for any direct or indirect loss, injury, or damages resulting from participation in coaching services. Clients agree to release and hold harmless the coach from liability arising from decisions or actions taken by the client.
6. Termination of Agreement
Either party may terminate the coaching relationship at any time with written or verbal notice.
  • Any unused, prepaid sessions (if applicable) will be handled according to the agreed-upon payment terms at the time of purchase.
  • Termination does not entitle the client to guaranteed refunds unless otherwise specified.
7. Certifications & Ethical Standards
The coach is currently in training through an NBHWC-aligned health and wellness coaching program and operates within the scope and ethical guidelines of recognized professional coaching standards. Coaching services are provided as non-clinical, non-therapeutic support.
8. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Vermont, without regard to conflict of law principles.
9. Acknowledgment & Consent
By participating in coaching services, the client acknowledges that they have read, understood, and agreed to the terms outlined in this agreement.
