Coaching Agreement
I understand that coaching is a collaborative partnership designed to help me clarify goals,
develop strategies, increase accountability, and support personal growth and healthy
relationships with self-first and then others. I understand that I am responsible for my own
decisions, actions, and results. I understand that coaching does not guarantee specific
outcomes. Coaching Is Not Therapy I understand that Visions Made Manifest Coaching Services
provides coaching services only. I understand that my coach is not acting as a licensed mental
health therapist, psychologist, psychiatrist, or medical provider. Coaching is not intended to
diagnose, treat, or cure mental illness. If I am experiencing a mental health crisis, suicidal
thoughts, severe depression, substance abuse, domestic violence, or another condition
requiring clinical care, I understand that I should seek assistance from an appropriately licensed
professional or emergency services. Confidentiality Information shared during coaching sessions
will remain confidential except when disclosure is: Required by law Authorized in writing by the
client Necessary to prevent imminent harm to the client or another person Cancellation Policy
Clients are asked to provide at least 24 hours' notice to cancel or reschedule an appointment.
Missed appointments without notice may be subject to a cancellation fee. Consent By signing
below, I acknowledge that: I have read this document. I understand the purpose and limitations
of coaching. I voluntarily agree to participate in coaching services. I understand that I may

discontinue coaching at any time.
